What Is Periodontal Disease?
Periodontal disease is a bacterial infection of the tissue and bone that hold your teeth in place. It begins as plaque along the gumline, hardens into tartar, and the resulting inflammation gradually separates gum from tooth. That separation creates a pocket, the pocket collects more bacteria, and the cycle deepens while the surrounding bone dissolves.
It moves through four recognizable stages:
- Healthy. Pockets of one to three millimeters, no bleeding, no bone loss. Six month cleanings are enough.
- Gingivitis. Redness, swelling and bleeding, pockets still shallow, bone intact. The only fully reversible stage.
- Early to moderate periodontitis. Pockets of four to six millimeters with visible bone loss beginning. This is where scaling and root planing is indicated.
- Advanced periodontitis. Pockets beyond six millimeters, significant bone loss, recession, sometimes loose or shifting teeth.

Why Maintenance Visits Are Every Three to Four Months
This is where patients push back, reasonably, because it looks like being asked to come in more often and pay more. The distinction is clinical.
